We accept Medicare and most major insurance carriers. For those that will be paying out of pocket for their care due to lack of insurance or high deductibles, we offer cash pay rates and payment options so that you can get the care you deserve!
In addition, you may qualify to participate in one of Berkana’s clinical trials, which may allow you to receive therapy at minimal to no cost.

ADMISSIONS PROCESS
A referral is not needed for you to see a physical therapist. However, some insurance companies may still require a referral from a physician. If you do need a referral, we will work with you to obtain one from your primary or specialty physician. We can help you determine if your insurance plan requires one. Just call us at 970.797.2431 to find out.
If you know that your insurance does not require a referral, you can enroll directly by calling our office at 970.797.2431 for an appointment.
Right to receive a “Good Faith Estimate” of outpatient rehabilitation costs
It is our goal at Berkana to provide you with the most accurate information we can regarding what the cost of your therapy will be based on your insurance. When you register as a patient, we will collect your insurance information. This information is then used to check your benefits, including whether a referral and/or authorization for treatment is needed and if there are any co-payments/insurances. Therefore, it is important to provide us with the correct information.
During your initial examination, your therapist will discuss the recommended plan of care needed to address your pain/limitations. This includes frequency of treatment and estimated duration of care. The therapist will then discuss the insurance information regarding copays, deductibles etc. that we obtained from your insurance company so that you understand what the potential cost of care will be. If you do not have insurance, your therapist will discuss the cash-based rate with you. This is always an estimate since the information provided by your insurance may change based on other claims that are in process as well as your overall response to treatment.
You can ask for a copy of this good faith estimate at the time of your initial evaluation or prior to starting your care.
